    Working under the guidance of the Director of Nursing (D.O.N.), the Resident Assessment Coordinator (RAC) leads a team that utilizes a core set of screening, clinical, and functional status elements to conduct a thorough assessment.

    The RAC uses this assessment to ensure a comprehensive care plan that addresses the resident's desires, requirements, and objectives.

    The ideal candidate for the Resident Assessment Coordinator role is a nurse with exceptional documentation and assessment abilities. They are detail-oriented, apply critical thinking, and follow the nursing process.

    The Resident Assessment Coordinator serves as an administrative nurse who assumes leadership and educational roles in a post-acute care environment.


    Key Responsibilities:
    - Coordinate the interdisciplinary assessment process for assigned residents
    - Develop and ensure completion of Comprehensive Care Plans in compliance with company, state, and federal policies
    - Monitor residents to ensure adherence to individual Care Plans and update them as needed
    - Highlight any discrepancies to the Director of Nursing and Care Plan Team

    Requirements:
    - RN (Registered Nurse) with an active State license
    - Prior MDS experience is a plus, but not mandatory as we provide training

    - Competence in computer usage and understanding of electronic documentation systems
