Nurse Practitioner House Calls Vermont - The Judge Group
Description
We are currently seeking (3) Nurse Practitioners to perform Health Risk Assessments (HRA) through house calls in Vermont. The position requires clinical documentation of acute, chronic, and long-term healthcare issues through the patient encounter in an EMR system.
As the provider, you will educate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence compliance. You will also collaborate with the supervising physician and other members of the healthcare team.
The fixed pay rate is $140 per in-person assessment with mileage reimbursement based on the current federal rate of 67 cents per mile. You may occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.
Qualifications include an active/unrestricted Vermont NP license, board certification by the ANCC or AANP, and a Family NP. New graduates will be considered.
