- Integrate primary care treatment plans with the interdisciplinary team
- Manage participant care in nursing homes and coordinate with specialists
- Collaborate with the interdisciplinary team for 24-hour care delivery
- Review medical outcomes and make operational changes as needed
- Ensure timely documentation and correct diagnostic coding
- Participate in on-call rotations when required
- Doctorate Degree (MD or DO) and active physician license
- Board Certification in Internal Medicine or Family Practice (Geriatrics preferred)
- Experience working with the elderly population
- Valid driver's license, insurance, and transportation
