    The mission of RecoveryWorks is to create access to opportunities for community, recovery and resource alongside our un-housed neighbors.

    Job Summary:

    The Recoveryworks Navigation Center is designed to provide emergency and case management services including access to showers, laundry, storage and mail for adults who are unhoused suffering from drug and alcohol addiction, mental illness and seeking recovery, employment, education and stable housing.

    The Case Manager provides comprehensive case management from time of enrollment through program completion and transition into community reintegration. The Case Manager is central in understanding every aspect of the client's case in order to continually move each individual through successful program completion and community reintegration. This includes linking guests to local community resources such as benefits, primary and behavioral health care, employment opportunities and stable housing. The Case Manager works closely with the Rehousing Manager in ensuring all aspects of programming are implemented and operational including milieu management, documentation and overall guest care. This position maintains a 25-40 person caseload. Additionally, the Case Manager is responsible for linking guests to already existing employment and vocational providers in the community, assisting in job development activities, including education; providing supportive employment services to all interested project participants, overseeing internal transitional employment positions within RecoveryWorks, and overseeing other special and/or short-term employment projects.

    Essential Job Functions:

    • Participate in intake, orientation and engagement of new clients
    • Provide ongoing engagement and assessment of guests in order to develop and update person centered, strengths-based Goals and Objectives (GO) plans.
    • Facilitate access and follow up with community based primary and behavioral health care providers.
    • Provide individual and group addiction recovery support.
    • Meet with every client on assigned caseload a minimum of 2x/month to assess progress, assist with daily and weekly schedule, review GO plan and schedule staffing as necessary.
    • Evaluate guests' needs through regular contact, including crisis intervention as necessary.
    • Manage each guest on assigned caseload from intake to community transition.
    • Provide regular group meetings with entire caseload to help build community and share resources.
    • Assist with managing the community milieu through regular, informal contact with guests both on and out of assigned caseload.
    • Provide backup case management coverage as needed.
    • Provides transportation for guests as necessary.
    • Acquire goods and services for clients based on determined need (e.g., housing, personal identification information, benefits acquisition, health insurance, food, medication, treatment etc.)
    • Participates in team and supervisory meetings as assigned.
    • Performs additional duties as assigned.
    Requirements:

    Education and Work Experience:

    • Bachelor's degree in social work, counseling or similar field preferred.
    • CAC II or III preferred.
    • Experience and interest working with disenfranchised persons.

    Knowledge, Skills and Abilities:

    • Knowledge, work history and/or education specific to case management, addiction, mental health, housing and homelessness.
    • Knowledge of clinical and evidence-based practices used to promote client recovery.
    • Skill in operating office equipment, including computers (including e-mail), software (e.g., Microsoft Word, Excel) and telephones.
    • Must demonstrate proficiency in all agency-wide database systems including HMIS.
    • Effective oral, written and interpersonal communication skills.
    • Able to prioritize multiple job responsibilities, work independently, and exercise good professional judgment.
    • Able to provide case management, education and peer group sessions to clients with substance abuse and/or mental health problems.
    • Able to advocate and negotiate in an assertive but tactful manner both internally and with community agencies, to have clients' need met.
    • Able to maintain client confidentiality.
    • Able to maintain a flexible work schedule, including some evening and weekend work.
    • Able to pass automobile insurance carrier's motor vehicle record investigation.
    • Must have own automobile, proof of insurance, valid drivers' license and ability to transport clients.

    Works primarily in a climate controlled office environment with frequent interpersonal interactions OR outdoors OR whatever the working conditions are.
