We specialize in delivering compassionate and personalized care to individuals in the comfort of their own homes. As we continue to expand our team, we are seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join us in providing high-quality home care services.

    Essential Job Functions of a Home Health Care Nurse:

    • Perform home health LPN (Licensed Practical Nurse) duties in patients homes
    • Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.
    • Develops and implements nursing care plans and discharge plans with involvement of patient and patient care team.
    • Review consumer medical documentation and or health referral forms, as relevant to the case, and adjust care plans are needed.
    • Health promotion, teaching, and training of family members
    • Understands and demonstrates commitment to the company's mission through daily communication with participants, families, community, and staff.

    Certificates, Licenses, and Registrations:

    • Current Massachusetts LPN Licensure
    • All state required training and certification completed in mandated timeframes.
    • Driver's License / Reliable means of transportation

    Qualifications:

    • Good Physical Assessment skills.
    • Must be able to use and understand electronic mobile devices for documentation and other electronic applications for the job
    • Effective Time Management
    • Travel Required
