    Job Title: Nursing Professional

    A compassionate and skilled Nursing Professional is needed to provide high-quality patient care in a designated group setting.

    Responsibilities:

    • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address their unique requirements.

    • Collaborate with interdisciplinary teams, including physicians and healthcare specialists, to coordinate and implement effective treatment plans.

    • Leverage leadership skills and clinical judgment to direct patient care units and delegate tasks as necessary.

    • Evaluate the quality of nursing care provided, identifying areas for improvement and implementing changes as needed.

    • Train and supervise home health aides, homemakers, and personal care workers in patient homes, recommending personnel adjustments as necessary.

    • Assess the need for medical equipment and supplies, recommending rentals or purchases to support patient care.

    • Maintain ongoing evaluation of equipment, reporting defects, repairs, or additional needs to the Hospice IDG.

    • Participate in on-call coverage systems, serving as a liaison between inpatient and outpatient services.

    • Engage in quality improvement initiatives, contributing to the development and implementation of best practices.

    • Demonstrate physical stamina for standing, walking, and lifting objects up to 200 pounds, with occasional sitting and lifting assistance for heavier objects.

    Requirements:

    • Graduate from an accredited School of Nursing, with a Bachelor's Degree in Nursing preferred.

    • Current license to practice as a Registered Professional Nurse in New York State, with specialized certifications as needed.
