We are looking for a dedicated Nurse Practitioner to join our healthcare team for wellness visits in and around Merrillville.

5K Sign on Bonus for Nurse Practitioner

Full Time - Monday- Friday

About the Position:

Conduct Adult/Geriatric Assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, and clear documentation
Collaborate with healthcare professionals and build relationships with advanced practice providers
Provide services in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits

Must Haves:

Master's Degree or equivalent experience with NP licensure
Current RN and NP licensure with prescription authority
Board certified by recognized bodies in an approved specialty
Minimum of 1 year experience as a Nurse Practitioner
Strong computer skills and communication abilities
Flexibility and willingness to travel
Valid driver's license and automobile insurance

Monday – Friday, Full-time position with flexible hours #TSNP

Job Type: Full-time

Pay: $102, $127,100.00 per year

Standard shift: Day shift

Weekly schedule: Monday to Friday

License/Certification: Certified Nurse Practitioner (Required)

Ability to Relocate: Merrillville, IN: Relocate before starting work (Required)

Work Location: On the road
