Requisition #660751 Location: Johns Hopkins Hospital Baltimore MD Category Support Services Schedule Day Shift Employment Type Full Time Join a fantastic global healthcare organization Employee Dependent Tuition Assistance Career growth development Affordable comprehensive benefits package Position Summary The Care Management Assistant is an integral member of the care management team working directly with patients families vendors to set up services implementing plan of care These activities include referrals data entry into Navihealth application for post acute placements transportation arrangements scheduling post discharge appointments primary specialists clinics organizing maintaining consent forms clerical activities needed Shift Full time hours per week Remote Job after completion of onsite office training Knowledge Skills Abilities The Care Management Assistant collaborates with social worker case manager to facilitate appropriate placement rehabilitation facilities alternate level care Develops knowledge services offered various facilities Meets patient family discuss recommendations rehabilitation facilities offers choice enters referrals into Navihealth application Obtains confirms payment authorization information Confirms acceptance communicates patient family team Documents work EMR according template Demonstrates ability obtain care Telligen completes PASSR screen post acute placement After discharge provide information discharge packet accompany patient facility Works directly patient family ensure that any post discharge appointments meet clinical needs directed provider social worker case manager fit into patients familys schedule Obtain appointment provider name timeframe appointment reason work Meet explain appointment type needed reason timeframe Arrange appointments meet expectations within timeframe Offer warm transfer between transition assistant provider office make appointments encourage self management help learn process Document appointment including provider phone address date/time EMR Arranges transportation via transition plan If going facility will setup pickup home cab Social Work Education High School Diploma required Associate degree health science closely related field preferred Experience Minimum one year previous hospital long term care experience working admissions Salary Range Minimum hour Maximum hour Compensation commensurate equity experience roles scope responsibility}
