- Visit preparation for all provider visits. Includes obtaining necessary records (medication lists, vitals, etc.) from each site for scheduled patients and using this information, rooming the patients prior to visit in Epic.
- Coordination of any visit follow-up needed after providers round
- Manage Epic in basket communication between site building nursing staff and Senior Transitions team.
- Process prior authorizations for patient prescriptions as needed.
- Coordinate communications with other specialties involved in the patient's care.
- Work is done in a hybrid model which includes time at the sites we serve as well as time working from home.
- 72 hours per 2-week pay period.
- Day hours with varying start and end times.

- Graduate of an accredited school of nursing.
- Current licensure as a licensed practical nurse in the State of Minnesota.
- Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
- Reliable transportation, Minnesota driver's license, a driving record in good standing, and the minimum amount of insurance liability coverage required.

Job DescriptionFind your purpose as a Licensed Practical Nurse within our Senior Transitions team
CentraCare Senior Transitions offers whole-person primary care in assisted living communities, transitional care units, memory care, and long-term care settings. The goal of our team is to care for patients in whatever senior community they call home as well as support those that are spending time in the transitional care units to increase strength and return to their homes. The CentraCare Senior Transitions team partners with community staff, families, and patients to provide routine primary care, acute illness care (such as pneumonia), and manage chronic medical conditions (like diabetes). The team also helps identify needs for things like home oxygen, medical equipment, home health services, and specialty medical care and help coordinate the setup of these things. The team sees patients in the St. Cloud area as well as CentraCare's western and southwestern regions.
