This assignment offers a weekly blended rate of $2,550, based on five 8‑hour day shifts. Ideal for an RN focused on high‑quality discharge planning, resource coordination, and effective continuity of care.

    Key Responsibilities of the Case Management RN

    Assess patient clinical, psychosocial, and discharge needs
    Coordinate discharge plans including home services, rehab, SNF, DME, and follow‑up care
    Communicate with physicians, therapists, social services, and external providers
    Conduct utilization reviews to ensure appropriate level‑of‑care
    Provide family and patient teaching regarding care plans and transitions
    Maintain accurate and timely EMR documentation
    Support safe, efficient, high‑quality patient flow

    Requirements of the Case Management RN

    Active Illinois RN license
    Current BLS certification
    Minimum 1–2 years' recent RN Case Management experience
    Excellent critical‑thinking and organizational skills
