    Description

    About the Position of Physical Therapist:


    We are looking for a PT - Physical Therapist to provide direct rehab care in the field of physical therapy to patients upon physician referral, ensuring compliance with all relevant laws, regulations, and Life Care standards.


    Educational and Experience Requirements:

    • Graduate of a recognized physical therapy program with a degree in BSPT, MSPT, or DPT (for those graduating in 2003 or after, an MSPT or DPT is required)
    • Currently registered/licensed in the applicable State and must maintain an active license in good standing throughout employment.
    • Preferred minimum of one (1) year experience in post-acute care or a related setting
    • CPR certification required upon hire or to be obtained during orientation, and must remain current during employment.

    Specific Job Responsibilities:

    • Engage in community health projects as deemed appropriate
    • Proficiency in Microsoft Word, Excel, and email usage
    • Collaborate with patients, families, and support departments for effective patient care planning
    • Practice good body mechanics consistently
    • Exercise autonomy in decision-making when necessary
    • Up-to-date on practices, procedures, laws, and regulations governing functions within the post-acute care facility
    • Implement and interpret departmental programs, goals, and policies
    • Demonstrate competency in all areas including patient rights, safety, and sanitation
    • Foster professional relationships with associates, vendors, etc.
    • Maintain confidentiality of sensitive information
    • Adhere to company policies, including compliance and anti-harassment procedures
    • Uphold integrity and professionalism following Life Care's Code of Conduct and mandatory compliance training

    Essential Duties:

    • Execute physical therapy treatment plans for patients
    • Establish, assess, and adjust realistic, measurable, and timely goals
    • Supervise and evaluate care provided by PTAs, Rehab Aides, and PT/PTA students
    • Maintain accurate and timely charting
    • Properly utilize therapy software
    • Provide excellent customer service and maintain a positive attitude towards patients
    • Assist in patient evacuations
    • Ensure regular and dependable attendance
    • Demonstrate sound reasoning skills and good judgment
    • Communicate effectively within an interdisciplinary team
    • Perform a variety of physical tasks during working hours
    • Demonstrate proficiency in English language skills
