- Provides assessment, evaluation, and management to patients that may include but is not limited to minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.
- Collaborates with patients to review their health background and wellness provide health counseling and education to determine physiological, emotional, and environmental needs, using this information to advise patients and their families on future health plans and illness prevention.
- Provides follow-up services as appropriate to assigned specialty, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other non-pharmacological treatment modalities.
- Maintains and reviews patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
- Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
- Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.
- Graduate of a master's level nurse practitioner program OR from and accredited physician assistant program. Required
- 1-3 years work experience in a clinical setting within a health care organization Preferred
- Understanding of the specialty department's information and techniques necessary for diagnosing and treating human injuries and diseases, including symptom recognition, treatment methods, medication properties and interactions, as well as preventive healthcare practices.

- Individual must posses either the licenses/certification details of a Nurse Practitioner or a Physician Assistant. ALL are required to have CPR/Basic Life Support.
- Nurse Practitioner: Registered nurse in the state of Wisconsin with a certification through American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioner Certification Board (AANPCB). Also certified in the State of Wisconsin as an Advanced Practice Nurse Prescriber or certification eligible, obtaining certification at the first opportunity Upon Hire Required
- Physician Assistant: Licensed by the state of Wisconsin as a Physician Assistant. Certification from the National Commission on Certification of Physician Assistant. Upon Hire Required
- CPR/Basic Life Support for Healthcare Provider certification from approved program. Upon Hire Required
