- Provides comprehensive nursing care and care management services to patients based on a nursing history/assessment, facility accepted clinical standing orders, policies/procedures and the physician's medical plan of care. Participates as a crucial member and leads the care team.
- Utilizes the nursing process in identifying individual patient problems and in developing, implementing and evaluating patient care plans.
- Communicates the patient needs and recommended treatment to care team.
- Triages patients who call or walk-in and directs them appropriately.
- Provides patient education and self-management support by making weekly/monthly calls to encourage patient to meet goals.
- Other related duties as assigned.

Summary
This position will support the Federal Government's response efforts to COVID-19.
This position is located in the White Earth Health Center, Ogema, MN. The purpose of the position is to provide professional nursing care management services to patients in an outpatient clinic setting. The incumbent of this position will report to the Director of Nursing or designee.
