    Description

    Summary

    The Assertive Community Treatment (ACT) team is dedicated to providing mental health rehabilitation services, supporting adults and families to reduce hospitalization and crisis episodes. The ACT Clinical Case Manager, operating under the guidance of the ACT Team Program Manager, delivers essential services, primarily in the clients living situation, including crisis intervention, case management, individual coaching, monitoring for medication adherence, and skills training. In the community and on call situations, the case manager exercises the experience and judgment required to make sound clinical decisions independently. Committed to ethical, culturally competent diverse and inclusive practices, the case manager ensures client service delivery aligns with regulatory, accreditation and best practice standards for optimal patient centered customer service satisfaction.

    Required Minimum Knowledge, Skills, and Abilities ( KSAs )

  • Education: Bachelor's degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, corrections, rehabilitation, nursing or closely related field is required. 2+ years of experience in the same areas of specialization; or 5 years of experience working with people diagnosed with severe mental illness. License/Certifications: MHRT-C certification required unless clinically licensed; Eligibility for LSW licensure preferred; Valid Driver's license and appropriate level of automobile liability insurance. Experience: 5+ years of experience working with a broad spectrum of mentally ill clients is preferred.
