    Vetted is seeking a Social Worker for a travel job in Happy Valley, Oregon.

    Job responsibilities:
    Conduct psychosocial assessments and identify barriers impacting care transitions.
    Document assessments, plans, and communications in the medical record.
    Coordinate discharge planning activities and communicate plans with patients and care teams.
    Screen for social determinants of health and incorporate findings into care plans as applicable.
    Work 40 hours per week with 8-hour shifts.
