Foster Care Case Manager – Jeannette, PA Join a Team That Transforms Lives Adelphoi Village is seeking a compassionate and qualified professional interested in working with siblings along with a foster family in a foster care setting Our agency-owned home is in Jeannette, PA that has a dedicated foster parent temporarily caring for sibling groups in need of long-term foster care placement Why Choose Adelphoi For over 55 years Adelphoi has partnered with local agencies and the juvenile justice system to deliver outcome-based services that empower at-risk youth Headquartered in Latrobe PA our team of more than 700 professionals provides residential community and educational services to youth in need As an Adelphoi employee you'll receive comprehensive training and strong supervisory support We provide collaborative strength-based work environment flexibility autonomy as well as opportunities for advancement within the program and beyond What will the job be like Provide ongoing support and deliver case management services for up to 6 youths in residence with an on-site office located in the Jeannette home with the possibility of other youth cases assigned
Develop individualized service plans to inspire emotional intellectual social growth Build relationships while setting boundaries for the foster parent children under care birth family children Youth Services court-related authorities Respond to crisis situations provide immediate assistance when needed Maintain accurate records daily basis documenting communications writing reports developing Individual Service Plans documenting casework session notes any required documents or workflows Client Module Sustain regular communications involved clients Assess Foster Family environment physical psychological safety Provide trainings maintain paperwork annual re-certification Resource Parent Module Electronic Health Record Assist family visitation Manage Monday through Friday schedule flexibility school needs occasional evenings weekends
