- Obtains medication histories for new patients and documents them in the electronic medical record in a timely manner.
- Medication history will be obtained from patient, patient's family, pharmacy, physician office, or long term care facility. Employee must be diligent to fully explore the medication history.
- Communicates with pharmacist appropriately including asking questions, and referring for additional interview or intervention.
- Uses good clinical judgement to update the medication history to the best possible list.
- Demonstrates knowledge and skills to appropriately communicate and interact with employees, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs.
- Included within this role assignment is the responsibility to be a positive role model for all peers always reflecting the WE CARE philosophy.
