- Meet with the patient and family to review hospice services and philosophy and obtain consents for care
- Develop an initial care plan that establishes immediate goals and interventions, based on nursing diagnosis
- Communicate with hospice staff members and other facilities as needed to coordinate optimal care and use of resources for patient / family members
- Provide direct nursing and supportive care for the terminally ill patient

- Valid Registered Nurse License
- BLS Certification

Mountain Valley has an opening for a Registered Nurse in the Dobson and Elkin service areas to assist with admitting new patients to hospice care. This position will complete the admission process and develop an initial plan of care.
