We are seeking qualified behavioral health professionals who will provide behavioral health assessment intervention consultation to individuals groups identified as having behavior needs plans treatment modalities coordinates follows treatment plans Will provide crisis evaluation intervention needed QBHP recognized Qualified Mental Health Professional under Medicaid Community-Based Mental Health Services Rule 140 Demonstrates ability apply critical thinking problem-solving abilities range client situations accurately assess diagnose develop treatment plans interventions clients possesses interpersonal skills professional attitude understands ethical standards behavioral healthcare Able interpret rules regulations good written oral communication skills plan organize work recognize signs suicidal behavior assess lethality respond clinically appropriate mannerResponsibilities
Performs diagnostic assessments collects diagnostic information determines immediate treatment plan needs patients Requests medical consultation indicated diligent timely manner Completes treatment plan conjunction patient identifying goals objectives connected needs identified assessment Provides behavioral health therapy individuals families groups identified having behavior needs Coordinates Emergency Response Service other crisis service teams clients presenting danger self others Maintains records accordance UnityPlace policies state federal oversight agencies accreditation agencies Joint Commission Medicaid Medicaid Rehabilitation Option Illinois Department of Public Health Commercial Insurances Acts liaison resource community patient needs coordinates treatment goals community agencies primary care physicians court system managed-care entities Meets clinical productivity standards Meets compliance standards documentation clinical charts Participates in-service competency training Prepares special reports activities work group activitiesAbout Us
