    Job Description:

    Under the direction of physicians, the Nurse Practitioner (NP) Heart Care Coordinator utilizes advanced clinical and analytical skills to provide direct care and management of heart failure patients.

    The Heart Care Coordinator demonstrates strong leadership, is an excellent communicator, and is effectively able to prioritize a large number of patients in various stages of heart failure.

    # Problems encountered have a significant level of complexity due to clinical and logistical issues.

    Job Duties:

    Works autonomously providing direct patient care including monitoring and assessing high risk patients.# Prepares and analyzes clinical data and collaborates with other medical personnel to facilitate meeting patient and family needs.# Coordinates preparations for any necessary surgical or interventional procedures.# Formulates a treatment plan which includes ordering diagnostic tests, selecting appropriate pharmacologic and non-pharmalogic interventions, develop a patient education plan, and obtains appropriate consultation or referral.

    Consults with physician appropriately regarding patient care issues and concerns.

    Evaluates operations from a workflow perspective and responds appropriately, redesigns per patient needs and competitive benchmarks.# Collaborates in the development of protocols and guidelines for patient management.# Leads in the evaluation, development and implementation of comprehensive patient education programs.## Seeks opportunities to flex costs downward when necessary.# Maintains knowledge of current practices in heart failure management.# Participates in professional organizations in the area of heart failure management, cardiovascular nursing and other areas of professional interest.

    Serves as a resource person for staff caring for the heart failure population.

    Accepts NPP students and works with to develop clinical skills pertinent to heart care patient management.# Assures that the program is in compliance with the appropriate governmental agencies.# Develops and presents information to community and professional groups on heart failure management.# Compiles information and data for the organization as requested.# Provides patient status reports to the community and referring physicians.

    Refers patients to heart failure studies.

    Provides education to patients and families regarding heart failure, management of complex heart failure drug regimens, and monitors patients# clinical course and stability.# Develops working relationships critical to the organization including patients, families, coworkers, other departments, physicians/providers and community.


    Minimum Requirements:

    Master#s degree in nursing and maintain board/association certification Licensed by the state of Missouri as an Advance Practice Registered Nurse Minimum 3 years in cardiovascular nursing (critical care, EPS, Cath Lab, heart failure experience preferred).

    Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) from American Heart Association (AHA) required CCRN and CHF certification by the Heart Failure Nursing Certification Board (CHFN) is preferred.

    Proficiency with electronic health records and database management.# Excellent written and verbal communication skills. Knowledge-Building/Sharing, Coaching/Empowering, Health System Vision # Management.
