Join our team as a Physician Assistant conducting comprehensive health assessments during home visits, skilled facility visits, and telehealth sessions.

Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment
Collaborate with physicians, case managers, social workers, and other medical personnel
Work in various environments including home visits, skilled facilities, and telehealth
Strong computer skills and familiarity with EMR software

Travel Requirements:
Ability to travel with a valid state driver's license and access to a car
