    Job Description

    For more than a century, Catholic Charities Brooklyn and Queens has been dedicated to delivering top-notch social services to the communities of Brooklyn and Queens. Presently, the organization provides over 160 programs and services catering to children, youth, adults, seniors, and individuals facing mental health challenges.

    STATEMENT OF THE JOB:

    • Responsible for coordinating care for children enrolled in various Health Homes Programs.
    • Engage and retain members in care by coordinating services and supporting adherence to treatment recommendations.
    • Develop individualized care plans in collaboration with clients, families, and service providers.
    • Promote wellness and prevention through resource linking based on individual needs.
    • Utilize electronic databases to share information and monitor outcomes.
    • Complete necessary documentation and maintain accurate records.
    • Provide transitional care and coordinate services for members' holistic well-being.
    • Ensure timely follow-up and comprehensive care for members post-discharge.
    • Establish and maintain networks with healthcare providers and community services.
    • Respect data security protocols.
    • Refer members to peer supports and self-care programs.
    • Provide direct services based on comprehensive assessments and service plans.
    • Report issues promptly to relevant authorities.

    QUALIFICATIONS:

    • Bachelor's degree in social work, psychology, or related field, with two years of relevant experience. OR Professional degree/certification in healthcare.
    • Skills in crisis intervention, time management, and psychosocial rehabilitation.
    • Fluency in a second language is desirable.
