    Main duties of the job

    We will provide you the opportunity to :

  • Manage a caseload of children and young people with a learning disability who require support and intervention in both specialist and mainstream schools
  • Facilitate and deliver a range of assessments and brief interventions, working both independently and as part of a multi-agency team
  • Work in partnership with specialist services to build progressive care pathways and improve health outcomes for children and young people
  • Work in a variety of settings including schools, clinics and family homes
  • Work flexibly and creatively to best engage and meet the diverse health needs of the school-aged population
  • Participate in safeguarding as per local and national guidance
  • Provide professional consultation to other professionals

  • To provide care to children & families with learning disabilities and/or complex health needs in a variety of settings
  • To participate in and support qualified team members in health promotion activities and/or social prescribing
  • To liaise with children and families; other professionals to ensure effective communication of care
  • Safeguard the client group by adhering to child and adult guidelines and procedures; this may involve managing situations where conflicting opinions are expressed
  • Actively participate in and support the development of evidence based practice and clinical
  • audit in collaboration with lead professionals and managers
  • Responsibility for policy and service development implementation
  • To follow organisational policies, procedures, processes and requirements, relevant to the role and comment on suggested changes where appropriate
  • Work in partnership with specialist services to build progressive care pathways and improve health outcomes for children and young people
  • Ability to take on the role of link nurse for appropriate subject, ensuring audits are completed and results/actions plans are disseminated
