One year of experience in the field of developmental disabilities;

Supervisory experience:

A personal vehicle that is insured and meets state safety/licensure requirements;
A valid driver's license may require a chauffeur's license;
Must successfully pass criminal background and Employment Disqualification List checks prior to working with consumers;

The main focus of this position is to coordinate the development and implementation of supported living services for individuals with developmental disabilities and employees, ensuring the delivery of quality services.
This coordination occurs through the direct supervision of employees, support for consumers, and ongoing communication with family, community, funding, and licensure sources.
All coordination must be conducted within the contract, state, and federal regulations, as well as Company Policies and Procedures.
All candidates must be able to work in a fast pace environment, be detailed oriented, be a team player as well as work independently, and have great problem-solving skills.

Responsibilities:Person-Centered Planning and Individual Support Planning:

Actively participate in the person-centered planning process, including the development and implementation of Person Centered Plans (PCPs) and Individual Support Plans (ISPs)
Design and implement support and teaching strategies targeted at assisting consumers with reaching their outcomes, as outlined in the consumer's ISP
Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP, and teaching strategies
Evaluate and revise support and teaching strategies as necessary to ensure progress toward the achievement of outcomes and action steps

Quality Assurance:

Review all forms, notify the Case Manager of any incidents, and provide oversight for the follow-up plan with staff
Maintain communication with family members, guardians, staff and Case Manager on a regular basis, particularly regarding policies, issues, and concerns

Supervision/Training:

Coordinate in-service training that is needed to assist staff in fulfilling responsibilities related to consumer-directed support
Provide direct support by providing services needed to protect the health and safety of the consumer, especially in a staffing crisis
Communicate with the Interdisciplinary Team about concerns and findings
Coordinate and assist staff with maintaining positive relationships
Be available to respond to staff in case of an emergency
Perform on-site evaluations for all field staff
