Responsible for working with a specific group, collaborating with the health care team to coordinate cost-effective services according to the member's care plan, promoting independence, and improving quality of life.
Essential Responsibilities:
Includes problem identification, psychosocial assessment, financial guidance, connecting with community resources, care placement, legal assistance, and support to other health professionals.
Schedule: Every other weekend and Holiday Week-1 Sun/Tue/Wed/Thurs/Fri Week-2 Tue/Wed/Thurs/Fri Basic Qualifications: Experience
Minimum 3 years of clinical experience and 2 years in case management.
Minimum 1 year of experience with the specific group.
Education
Must have a Masters in social work (MSW).
License, Certification, Registration
Multiple state credentials are required.
Several licenses required within specific timeframes post-hire.

Preferred Qualifications:
Experience with Windows software programs preferred.
Familiarity with community systems and resources in the service area preferred.
Understanding of regulatory issues in the Mid-Atlantic area preferred.
Previous clinical social worker experience preferred.
Notes:
Will be covering Inpatient and Emergency Department situations.
MSW qualification is a must.
