    We are looking to hire a dynamic and passionate Case Manager who is client centered, community focused and is a self-starter.


    Position Summary

    LifeSpring is a growing program that assists youth and young adults who are exiting the foster care system as well as youth and young adults who are experiencing homelessness.

    Under the general supervision of the LifeSpring Program Manager, assures that all services provided to LifeSpring participants are being implemented through a youth driven, wellness and recovery model that is strength based, culturally relevant, trauma informed, and outcome based that supports self-sufficiency.

    The Case Manager provides intensive case management and independent living skills development to the participants. Assist youth indeveloping skills, obtaining employment, pursuing educational goals, and developing life plans that will supportindependence. Will conduct intakes, eligibility screenings, and basic needs assessments. In addition, will assist with housing navigation. Possess knowledge and expertise in relationship to transitional age youth, specifically former foster youth, CSEC/HT and homeless youth, and the specific issues surrounding this population and demonstrateexcellent engagement skills.

    Provide skill development opportunities for youth including life skills training, meditation, economic literacy, transportation assistance, public benefits advocacy, financial assistance, and tenancy training. Provide supportive services to parenting youth including individualized housing, assistance securing childcare, education and support, maternity/paternity provisions, and specialized services for children. Provides linkage to services within the community to support transition to independence as well as aftercare, including but not limited to, mental health services, educational (college or post-secondary training) and employment support, etc. Provide crisis management and safety planning to the youth in the program.

    Assist youth and young adults in developing and maintaining permanent connections. Participation in county collaborative meetings to identify service gaps, barriers,and successes in order to create best practices, policies, procedures, and protocols. Provide training and outreach.


    Required Education and Experience

    • BA + 2 years experienced required or MA in social work or related field and registered intern with the BBS preferred.
    • Experience working with teens and/or transitional age youth who are experiencing homelessness or who are at risk of homelessness. Experience working with other populations such as former foster youth and CSEC/HT.
    • Ability to work with minimal supervision and focus on multiple duties. Strong skills in engagement, crisis intervention,case management, interviewing, rapport building, assessment, goal setting, and youth participation and development.
    • Excellent verbal and written communication skills and computer literacy required. Must be highly organized, detailoriented, and self-motivated.
    • Available for alternating evening and weekend on-call shifts for crisis intervention.
    • Available nights and some weekends for groups, events, and/or outings.

    • Possesses advanced knowledge that is primarily intellectual in character and includes exercising discretion and independent judgment

    Other Requirements

    Reliable transportation. Must have a California driver license and good driving record. Willing to use own, properly insured, reliable vehicle for the job (mileage reimbursed). Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.


    Specific Activities

    • Utilize therapeutic engagement strategies to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence.
    • Provide intensive and therapeutic case management services designed to build and support participants'relationships with family and community with an emphasis on permanent connection
    • Utilize evidenced based practices to engage youth in participation of services that are client-centered, culturally appropriate, trauma-informed, in order to improve the short/long-term health, safety, and well-being of youth.
    • Utilize evidence based or promising practices as the preferred treatment modality for crisis management and safety planning, as well as individual and group sessions.
    • Provide coordination of care with other mental health professionals, primary care providers, and other supportive services.
    • Coordinate and engage youth in activities in the community to stimulate alternative interests, improve emotional and physical wellbeing, enhance self-esteem, and foster personal growth and satisfaction.
    • Provide support, advocacy, life skills, referrals, crisisintervention, and employment readiness
    • Carry a reasonable caseload required by funding source and provide responsive services in the office, home, and/or community setting.
    • Participate in HOY Case Conferencing with partners
    • Workclosely with Housing Navigator and Peer Advocate to screen and interview prospective participants for thep
    • Will utilize the Coordinated Entry System to connect youth and young adults in finding safe, stable and inclusive housing options.
    • Work with youth and young adults to explore the 3 E's (Education, Employment, Entrepreneurialism) and assist in connecting them to the appropriate community partner so they can get support in the pursuance of their identified goals.
    • Assist youth in obtaining documentation, housing, education and legal services, etc.
    • Work with participant to develop a case plan and provide assistance when needed.

    • Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants.
    • Ability to work independently and use good judgment.
    • Work in conjunction with other programs to create and uphold to monthly events in house or offered through the community.
    • Complete all other duties as assigned.

    Work Schedule

    40 hours, full-time, Monday through Friday, onsite. Times may vary per individual client needs (may include evening and weekends). Available for alternating evening andweekend on-call shifts for crisis intervention.

    While performing the duties of this job, the employee may be exposed in an extremely stressful environment wherethere are considerable mental and emotional demands. Travel is required to attend meetings throughout San DiegoCounty. Employee may be exposed to weather conditions prevalent at the time.
