    is looking for a pediatric/school based Physical Therapist to provide therapy services within the students home in Coopersville, MI Student requires 30 min 3 times per month of services.

    Homebound child. Services to begin

    30 minutes per session, 3 times per month


    Rate:
    $75-$80 per hour direct services only+ round-trip mileage at IRS rate

    Virtual meetings billed at $60/hour

    Cancellations/No-Shows billed half of session time


    Start Date:

    ASAP

    Scheduling:
    Flexible—PT to schedule with directly with caretakers, services can be provided during school, after school, evenings and weekends


    Assessment Materials:

    Requirements

    Active Physical Therapist license in Michigan

    Comfortable conducting in-person therapy sessions at students home

    Pediatric experience (required)

    School-based therapy/IEP experience (preferred)

    Personal computer with high-speed internet

    Distraction-free workspace
