Home Health Intake and Scheduling Coordinator - Charlotte

+ Verify insurance coverage and obtain necessary authorizations
+ Coordinate home health services for clients
+ Work closely with patients, families, clinicians, and insurance companies to ensure timely service delivery.
Responsibilities include intake of new patients, scheduling appointments with clinicians, maintaining accurate records of client information and services provided. The coordinator will also communicate benefit information with referral sources as necessary and maintain excellent relationships with clients, teammates, and providers. This is a fast-paced role in a growing company working with a very friendly and dynamic team.
