    Description

    Acts as the patient care team leader for determining patient care needs and following up on the patient plans of care while ensuring communication with patients and their families across the primary care team. Lead the patient care coordinators in the six specific activities of care coordination; determine and update care coordination needs, create and update a proactive plan of care, communicate, facilitate transitions, connect with community resources and align resources with population needs.

    Job Tasks:
    • Oversees the bi-monthly patient care team meetings
    • Performs NDC number verification
    • Tracks faxes and electronically imports documents into patients charts.
    • Owns Orders tracking for assigned location
    • Oversees centralized shot records
    • Follows up on all hearing screens and takes appropriate action.
    • Collects and analyzes data to assist with documentation, clinical care, and positive patient outcomes
    • Completes data logger temperature review
    • Follow-up with laboratories and courier services regarding issues with specimen handling, pick-up, rejection and testing.
    • Responsible for ensuring care coordination needs are based on a patient's health care needs and treatment recommendations,
    • Follows up on plans of care for medically complex patients.
    • Ensures communication with patients and families
    • Updates care coordination needs, as required
    • Orders and ensures that assigned office is appropriately stocked with samples
    • Direct oversight of clinical equipment and supplies for assigned office.
    • Ensures adequate inventory in assigned office.
    • Initiates and maintains central ordering of vaccines, both VFC and private, for assigned office.
    • Works with preceptors to ensure a healthy learning environment
    • Works with preceptors to formulate performance improvement plans to assist medical assistants with the completion of their initial proficiency checklists
    • Performs newborn screenings - Coordinates necessary repeats, requests from DHEC and/or other state health departments; imports/routes to appropriate medical provider.
    • Organizes, plans, and manages time effectively.
    • Completes all mandatory training
    • Performs other duties as assigned.

    Requirements

    QUALIFICATIONS
    • Associates or Bachelors of Science degree in Nursing
    • Active Registered Nursing license valid for use in South Carolina
    • Minimum of 3 years of healthcare experience preferred
    • Outpatient practice experience preferred
    • BLS Certification
    • Prior experience with electronic medical records
    • Knowledge of OSHA standards
    • Working knowledge of computer applications
    • Ability to speak, read and write effectively
    • Ability to work independently without direct supervision
    • Ability to coordinate, analyze, observe, make decisions, and meet deadlines, in a detail-oriented manner

    • General office equipment such as telephone, copy machine, fax machine, scanner, personal computer.

    Physical requirements
    • Good visual acuity and ability to verbally communicate.
    • Ability to lift, lower, push, pull, and retrieve objects weighing a maximum of 25 pounds.
