    Welcome to the LPN Intake Coordinator Position!


    We are excited to introduce you to our LPN Intake Coordinator role at our company. The LPN Intake Coordinator acts as a vital link to referral sources, ensuring prompt response to agency referrals and coordination of services. This position entails managing the patient intake process for both home health and hospice care, ensuring seamless and efficient service delivery.


    Key Responsibilities:
    • Receive and request physicians' orders
    • Oversee the department to guarantee optimal patient care
    • Collect information from referral sources and verify insurance eligibility
    • Assist in assigning and scheduling clinicians
    • Evaluate referrals for suitability

    Requirements:
    • Active WA state LPN license
    • CPR/BLS certification
