    Position Summary:

    Supports psycho-social needs of patients who are currently receiving or recently received hospital based care. The Social Worker aims to enhance the level of health/recovery, provide interventions for a range of populations from infants to geriatrics, be a resource for the healthcare team, aid in identifying discharge planning needs, and offer support/intervention in challenging situations.

    Experience:

    Minimum two years post graduate experience as a social worker in a similar healthcare setting is highly preferred. Successful completion of a 6-month hospital-based orientation process is required. For staff caring for patients in New York, LMSW, LSW, or LCSW certification is required.

    Essential Functions:
    • Provides psychosocial evaluation to meet patients' medically related social needs.
    Such services include, but are not limited to:

    a. Assessing social and emotional factors related to the patient's illness, need for care, response to treatment, and adjustment to hospital care.
    b. Evaluating the patient's medical and nursing requirements, home situation, financial resources, and community resources for discharge decisions.
    c. Participating in unit interdisciplinary conferences to identify patients and families in need of intervention.
    d. Developing plans based on patient needs evaluation.
    e. Participating in a 24-hour on-call system.
    f. Receiving training to support trauma patients.
    g. Responding to consults as per policy.

    • Facilitate discharge planning to meet patient's continued care needs (e.g., skilled nursing, rehabilitation, etc.)

    a. Identifying psycho-social, environmental, and financial problems affecting discharge planning.
    b. Collaborating with the healthcare team to establish discharge plans.
    c. Consulting with Case Managers as needed.
    d. Keeping stakeholders informed of the plan.
    e. Completing required documentation related to transfers.
    f. Documenting patient status appropriately.

    • Documents reflect continuous progress in the care plan.

    a. Documenting the care plan fully as needed.
    b. Providing sufficient details for healthcare team members to stay informed and implement the plan.
    c. Timely and detailed entries for interactions/interventions.
    d. Complete documentation of handoffs daily.
    e. Including all regulatory components in documentation.

    • Assess and facilitate financial needs.

    a. Responsibly utilizing available charity funds according to procedures.
    b. Assisting patients/families with financial applications.
    c. Collaborating to meet financial needs.

    • Support individuals during crisis situations.

    a. Providing intervention for crisis situations across the hospital.
    b. Assisting with crisis interventions as needed.
    c. Providing interventions for high-risk patients.

    Other Duties:

    Performs other assigned duties.
