Job summary
The Connecticut Joint Replacement Institute (CJRI) is seeking a Patient Discharge Coordinator to plan safe patient discharge plans and arrange for in-home or outpatient services. The coordinator will partner with physicians and office staff to meet length of stay targets and ensure timely provision of services.
Qualifications
- Associates degree in related field preferred
- Three (3) years of healthcare experience
- Knowledge of Allscripts/EPIC
- Knowledge of medical terminology
