    Description
    Blize Healthcare is looking for an experienced Registered Nurse or Licensed Vocational Nurse to join our growing team.
    Job Position:
    • Registered Nurse or Licensed Vocational Nurse (Full-time or Per Diem available).
    • We're hiring in Solano County, Contra Costa County and Alameda County.

    Job Description:
    • Responsible and accountable for the delivery and coordination of patient care and the application of the nursing process, during after-hours and weekends.
    • Demonstrates the ability to make clinical judgments and utilize critical thinking in an effective and efficient manner with supervision.
    • Perform according to nursing standards and is accountable for managing patient care and assisting others in the management of patient care.
    Required Skills & Qualifications:
    • Must have a minimum of 1 year of experience as an RN or LVN.
    • Provide skilled nursing services/interventions in accordance with physician orders.
    • Assesses patient needs plans for evaluation and modifies care to meet goals of skilled nursing interventions.
    • Collaborates with all disciplines to plan and evaluate team goals for each patient.
    • Experience in home health or hospice is recommended.
    Job Types: Full-time and Part-time are available. COVID-19 Considerations:
    • We provide PPE such as masks, gloves, gowns, face shields, sanitizer, and alcohol wipes/Sani-Cloths.
