The Registered Nurse (RN) in Home Health delivers and oversees nursing care for patients at their homes following physician's orders and in compliance with relevant laws, regulations, and agency protocols.

Collaborating with healthcare colleagues to plan and deliver comprehensive care, ensuring continuity of patient care through communication with physicians, agency staff, patients, families, and community resources, all within a Point of Care setup.
Conducting initial and/or complete nursing evaluations, establishing the focus of care, creating the care plan within state regulations, and documenting accurately and promptly in alignment with physician directives.
Performing thorough patient assessments at specified intervals and devising suitable care plans.
Verifying that patients meet home healthcare criteria and medical necessity standards as set by payers.

License Requirements

Active RN license in the residing state
Valid CPR certification
Current Driver's License, auto insurance, and access to reliable transportation or public transit
