- Conducts initial and periodic occupational therapy assessments to determine PACE Participants' care needs, preferences, and goals. Establishes realistic and measurable goals and sets up treatment plans to administer rehabilitation treatment.
- Participates as an active member of the IDT for Participant care planning.
- Remains alert to pertinent input from other team members, Participants, and caregivers and updates the IDT promptly regarding any changes in Participants' condition or medical status.
- Oversees skilled and therapeutic exercises in the PACE Rehabilitation Gym ensuring that therapy conforms to accepted industry practice standards and organizational/department standards.
Provides supervision to program aides and other staff who are implementing occupational therapy treatment and/or maintenance programs. - Graduate of an Occupational Therapy program accredited by the American Occupational Therapy Association
- Valid and current California Occupational Therapy license in good standing
- Valid CA driver's license with good driving history (last 5 years)
- Current BLS and First Aid required.
