    POSITION SUMMARY:

    Sign-on bonus starting at $6,000 Plans, evaluate and administers physical therapy services and care to patients upon physician#s orders.

    Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.

    # Coordinates family involvement in assessment and treatment of the patient.# Responsible for both inpatients and outpatients as needed.


    EDUCATION/TRAINING:

    Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation).# Participates in educational programs on an ongoing basis and complete mandatory CEUs as required.


    LICENSURE/CERTIFICATION:

    Current license from the State of Illinois or in the process of acquiring licensure.#

    An applicant for employment as a physical, occupational, or speech therapist must present acceptable evidence of licensure or authorization to test at the time of employment.

    # An applicant who fails to provide this documentation will not be hired.# A new employee will not be allowed to work as a therapist until evidence of licensure eligibility is provided.# CPR certification required.


    EXPERIENCE/SKILLS/ABILITIES:

    Successful completion of annual clinical, age and job specific competencies and skill verification tools required.# Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.# Willingness to accept responsibility.# Ability to direct ancillary staff and organize work assignments.# Leadership skills essential to the position.

    Must be skilled in a broad range of modalities with a clear understanding of indications and contraindicating of each. Familiar with the indications and use of equipment. Guides, teaches, and supervises students.
