    Job Title: Case Manager
    Reporting Relationship: Case Management Supervisor
    Work Schedule: As per schedule (shifts and weekends); on call
    Exemption Status: Non-exempt
    Grade: I - IV
    Definition:
    The primary responsibility of the Case Manager is to provide guidance, direction, and management of reentrants and their activities. They will act as the primary contact for reentrants, assisting them with concerns and issues. The Case Manager must determine reentrants' social service needs and assist in creating Individual Renewal Plans.
