    CCTC is seeking a Care Coordinator for the Tamaa Program to support and follow through with treatment team recommendations in a fast paced environment and meet the needs of the children ages 2.

    5yrs to 21yrs and their families presenting with crisis and behavioral health issues in a trauma-informed manner

    Responsibilities include:
    Identify resources within the targeted community ( e. g., MH/MR Centers, schools, day care centers, District Health Centers, recreational programs, legal assistance programs, housing assistance programs), as well as other relevant resources located throughout Philadelphia, establish relationships with these resources, and keep up-to-date records about these resources.
    Establish and maintain a positive working relationship with West-African refugee community, cultural, and religious groups/leaders.

    Assess the service needs of refugee children and families and develop appropriate individualized service plans in collaboration with the children and families.

    Link refugee children and families to needed resources.
    Work w/ families to remove any barriers that may prevent participation in needed services (e.

    g., insurance)Work w/ refugee children and families on a regular basis (at least monthly face-to-face meetings) to assess progress and need for additional services/support.

    Revise children and families' service plans accordingly.
    Work collaboratively w/ other team members and external resources to ensure identified needs are being met/interventions are implemented.

    Maintain written/electronic records of all client and resource contacts (both telephone and face-to-face contacts).Organize, run, and keep records of Multicultural Community Events.


    Requirements:


    Bachelors degree in Social Work or a related field; A minimum of two years experience in working as a case manger or in a related position with children and families; Interest and experience in doing community outreach; Ability to work well as part of a multi-disciplinary team and with a culturally/ethnically diverse population; Strong interest and/or experience in working with refugees.

    Covid vaccination required.
