Virtus Healthcare Staffing is partnering with a leading PACE (Program of All-Inclusive Care for the Elderly) provider clinic in Stockton, California to find a compassionate and dedicated Nurse Practitioner - Psychiatric (NP) to join their team

In this rewarding role, you'll play a vital role in providing comprehensive mental health care to a population of older adults facing complex medical and psychosocial challenges. The PACE program offers a holistic approach to care, integrating medical, social, and behavioral health services under one roof.

About PACE:

The PACE program provides a comprehensive and coordinated care model for seniors 55 and older who are dually eligible for Medicare and Medicaid. This setting allows you to collaborate with a multidisciplinary team to ensure seamless care coordination and positive patient outcomes.

You'll Make a Difference By:

• Conducting comprehensive psychiatric evaluations for older adults with a variety of mental health conditions, including depression, anxiety, bipolar disorder, and dementia.
• Developing and implementing individualized treatment plans, incorporating medication management, psychotherapy, and referrals to other necessary services.
• Providing ongoing monitoring and adjustment of treatment plans as needed.
• Collaborating with physicians, social workers, therapists, nurses, and other healthcare professionals to ensure coordinated care and address the patient's full spectrum of needs.
• Educating patients and their families about mental health conditions and treatment options.
• Contributing to quality improvement initiatives and program development within the PACE model.

Qualifications:

• Master of Science in Nursing (MSN) degree with a specialization in Family Practice or Psychiatric Mental Health (PMHNP) from an accredited program.
• Active California State Nurse Practitioner license.
• Board certification in Psychiatric Mental Health (PMHNP) is preferred.
• Minimum 2 years of clinical experience in providing psychiatric care to geriatric patients.
• Strong clinical knowledge of adult mental health disorders and evidence-based treatment approaches.
• Excellent assessment, diagnostic, and treatment planning skills.
• Compassionate and patient-centered approach to care.
• Excellent communication, collaboration, and interpersonal skills.
• Ability to work effectively within a team-based, interdisciplinary healthcare environment.
