- Conducting comprehensive psychiatric evaluations for older adults with a variety of mental health conditions, including depression, anxiety, bipolar disorder, and dementia.
- Developing and implementing individualized treatment plans, incorporating medication management, psychotherapy, and referrals to other necessary services.
- Providing ongoing monitoring and adjustment of treatment plans as needed.
- Collaborating with physicians, social workers, therapists, nurses, and other healthcare professionals to ensure coordinated care and address the patient's full spectrum of needs.
- Educating patients and their families about mental health conditions and treatment options.
- Contributing to quality improvement initiatives and program development within the PACE model.
- Master of Science in Nursing (MSN) degree with a specialization in Family Practice or Psychiatric Mental Health (PMHNP) from an accredited program.
- Active California State Nurse Practitioner license.
- Board certification in Psychiatric Mental Health (PMHNP) is preferred.
- Minimum 2 years of clinical experience in providing psychiatric care to geriatric patients.
- Strong clinical knowledge of adult mental health disorders and evidence-based treatment approaches.
- Excellent assessment, diagnostic, and treatment planning skills.
