- Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment of medical conditions, to include follow-up and any problems detected on a schedule that consists of an average of 32 patient facing hours per week
- Manage acute and chronic illness, develop a plan of care, coordinate care, and determine the need for case conference
- Develop a comprehensive plan of care and make appropriate referrals to both internal disciplines of the clinic and/or external agencies for additional services required
- Regularly incorporate patient education into patient visits and monitor and direct the progress of patients with chronic disease
- Collaborate with all members of the health care team including behavioral health, dental services, and social work to provide holistic care to patients
- Responsible for accurate and timely documentation in the patient's electronic medical record and locking record in a timely manner as specified in policies and procedures
- Utilize evidence-based guidelines in practice
- Provide clinic consultation to physicians and mid-level providers as needed
- Guide and assist the compliance officer to maintain standards and assist annually with the review and revision of policies and procedures
- Work with community partners to ensure proper follow-up of clinic patients after hospitalization or mental health evaluation
- Provide clear written and verbal communications to patients and staff, maintaining a culture of dignity and respect
- Assist in planning to evaluate performance levels, quality of service, productivity and future needs
- Operate computer and electronic health records competently
- Must be able to receive, interpret and follow verbal and written communications
- Participate in various administrative/clinical staff meetings to include, but not be limited to, Quality Management Committee, clinic meetings, general staff meetings, case conference/management, and/or administrative meetings as requested
- Limited telephone only on-call services as needed (typically 3- 5 nights/month)
- Perform other duties that may be assigned by the CCO or members of the Executive Team
- Current Kansas license to practice medicine
- Current Basic Life Support certification
- N95 FIT test required
- Current Tuberculin skin test within one year or QuantiFERON Gold within two years
- Minimum one year of experience preferred, with a strong background in acute or primary care
- Computer skills and ability to navigate Electronic Health Record software
- An interest in community healthcare and underserved populations is preferred
- Good listening and communication skills with the ability to receive, interpret, and follow verbal and written instructions
- Ability to speak a foreign language is helpful, though not necessary
- Self-motivated

- Hours Monday – Friday ranging from 7:30am – And Saturdays as required
- Shifts are generally 8-10 hours in length of direct clinic care and Actual schedule shall be determined by the CCO

Job purpose
The Medical Doctor (MD) is responsible for providing comprehensive primary care and management of patients with acute and chronic issues in a collaborative care team setting. The MD sees patients independently, in collaboration with advanced practice providers, Behavioral Health Consultants, and those patients referred by other physicians. MD may grow their practice to meet the needs of the community in line with the MD's interests and passions. The MD plays a key role in increasing patient encounters and expanding the development of an integrated health care model for Heartland Community Health Center.
