- Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans based on assessment findings, patient goals, and physician orders.
- Monitor and evaluate patients' progress, symptoms, and response to treatment, adjusting care plans as needed to optimize outcomes.
- Clinical Care and Procedures:
- Provide skilled nursing care and perform clinical procedures including wound care, medication administration, IV therapy, and other treatments as prescribed by the physician.
- Educate patients and caregivers on proper medication management, disease management, and self-care techniques to promote optimal health and independence.
- Coordination of Care:
- Collaborate with interdisciplinary team members including physicians, therapists, social workers, and home health aides to coordinate and deliver comprehensive care services.
- Communicate effectively with healthcare team members to ensure continuity of care and facilitate timely interventions as needed.
- Patient and Family Education:
- Assess patients' and families' learning needs and provide education and support on disease management, medication compliance, lifestyle modifications, and other relevant topics to promote wellness and prevent complications.
- Documentation and Compliance:
- Maintain accurate and up-to-date clinical documentation in accordance with regulatory requirements, agency policies, and professional standards of practice.
- Ensure compliance with all applicable laws, regulations, and guidelines governing home health care services.
- Quality Improvement and Professional Development:
- Participate in quality improvement initiatives, performance improvement activities, and peer review processes to enhance the delivery of care and outcomes for patients.

Position Title: RN Home Health
Work Location: Eureka, CA
Shift: Days 40 hr/Shift
Shift Schedule: Day 5x8-Hour (08:00 - 16:30)
Ratios: 1:1
Locals Accepted: Yes; Considered local within 50 mile radius, will accept locals
Contract Duration:13 weeks
License Requirement: CA RN license
Certification Requirement: BLS
Years of Experience REQ: 1 YR of Skilled Home Health Experience
Pay Rate:$58-$64/hr
