OverviewOur client is seeking a full-time LPN.

The LPN will be responsible for providing direct patient care under the direction of a designated Care Manager and/or Central Monitoring Nurse.

This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality. The candidate should have experience in hospital and home care settings. Key Responsibilities

• Provide prescribed medical treatment and personal care services to patients in a home or inpatient setting
• Utilize principles of the nursing process to assist in data collection, planning and evaluation of nursing care
• Demonstrate nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care
• Take and record patients' vital signs
• Administer specified medication, orally or by subcutaneous or inter-muscular injection, and note time and amount on patients' charts
• Reinforce instructions and explanations of the patient Plan of Care or teaching plan
• Use oral and written communication to convey pertinent information to members of the health care team in a timely manner
• Report any significant changes in the patient's health status to the appropriate individual(s)
• Maintain a safe environment for the patient and self
• Identify unmet learning needs and seek assistance from others in meeting those needs
• Comply with company policies and procedures
• Complete all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe
• Recognize and follow established lines of communicationSchedule and Shift Details
• 8:30a-4:30p, 1 weekend per month
• EMR: Home Care Home Base
