- Evaluate, monitor, recommend and design patient specific pharmacotherapeutic regimens
- Participate in pharmacy activities to assure safe, effective, appropriate, and economical use of medications to improve continuity of care and outcomes
- Serve as a drug information resource to practice physicians and nurses and other team members
- Participate in health system collaboratives as well as liaison with the acute care facilities
- Engage in population health and wellness activities on behalf of the Clinically Integrated Network in collaboration with care coordination and managed care plans
- Develop medication use policies and procedures and create/expand chronic care management programs
- Create collaborative practice agreements to optimize evidence based pharmaceutical care
- Participate in quality improvement activities including patient advisory committee, and patient care committees
- Provide drug-therapy related education and training to team members
- Participate in problem-solving where appropriate about best practice development for medications such as high cost, or otherwise sub-optimal therapies
- Regularly monitor patient data and metrics to evaluate program effectiveness and clinical outcomes, and report updates
- Accept and perform other duties as assigned
- Doctor of Pharmacy degree from an ACPE-accredited institution and PGY1 pharmacy residency training are required
- Completion of a PGY2 pharmacy residency in ambulatory care or family/internal medicine is required or at least 3 years of practice related experience
- Pharmacy licensure in New Jersey will be required
- The successful candidate will possess a high level of clinical practice skills and the interpersonal skills needed for interacting effectively with students, health professionals, and patients
- Certification in basic life support (BLS) required
- Certification in Ambulatory Care (BCACP) by the Board of Pharmacy Specialties (or other relevant board certification) is expected within two years of hire

Job Summary:
An Ambulatory Care Clinical Pharmacist principal responsibility is to establish and maintain a direct patient care practice in the RWJBarnabas Health Rutgers Medical Group office practice site.
Comprehensive medication management includes but is not limited to:
pharmacotherapeutic interventions, pharmacy consultations, providing patient counseling and medication reconciliation, documentation of pharmacy activities, medication errors and adverse events.
