Responsibilities:
We are on the lookout for dedicated LVN's to join the skilled nursing teams at CSTCC. As an LVN within our dynamic team, you will be responsible for the care of a diverse range of patients, including those undergoing short-term and long-term rehabilitation. Your role will be vital in maintaining seamless patient care transitions between shifts, overseeing the care provided by CNAs, and supporting other team members.
In cases where patients require ongoing care, subacute care plays a critical role. Community Subacute & Transitional Care Center, a 106-bed facility, offers inpatient services for chronic, subacute conditions such as coma or complex illnesses necessitating 24-hour medical monitoring.
Patients needing ventilator, respiratory, wound, IV, hospice, rehabilitation services, or general nursing care can expect comprehensive treatment comparable to that of an acute care facility, all within the comfort of a long-term care environment.
