- Provide direct nursing care to residents under the supervision of an RN or as directed by physician orders.
- Administer medications and treatments per orders and facility policy.
- Take and record residents' vital signs including blood pressure temperature pulse respiration monitor for changes in condition assist residents with activities of daily living as needed including bathing dressing feeding mobility hygiene perform basic wound care dressing changes catheter or ostomy care collect specimens for lab tests observe document residents' physical emotional status report any changes to the supervising RN or physician maintain accurate timely documentation communicate effectively adhere to infection control protocols standards of care qualifications include valid LPN license in Connecticut graduate from accredited practical/vocational nursing program current CPR/BLS certification strong interpersonal communication skills ability to work well with team compassionate approach good organizational time-management skills comfort working long-term skilled nursing environment experience preferred.
