DUTIES OF THE MDS COORDINATOR:

• The MDS Coordinator is responsible for the coordination of patient/resident care from admission through discharge.
• Starts and monitors RAI process tracking, discharge/re-entry and tracking forms through the PCC system
• Follows up with staff to assure compliance to standards of documentation
• Collect data for each resident and interviews staff and residents as necessary to assure good standard of practice and as instructed in the MDS 3.0 User manual
• Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient's care needs and captures all resources utilized to ensure correct payment by Medicare/Medicaid and insurance programs
• Provides interdisciplinary schedule for all MDS assessments and care plans
• Participating in daily Interdisciplinary Team Meeting and other meetings such as PDPM meetings, Care plan meetings, Triple Check Meetings
• Establishing and maintaining positive relationships with all members of the healthcare team and will function as part of the nursing management team.

Qualifications:

• Must have a current, unencumbered LVN or RN license in Texas
• Must have a minimum of 1 year of direct hands-on nursing experience
• Must have a minimum of 1 years of long-term care experience
• Minimum 1 year of RAI/MDS experience
• Proficiency in ICD 10 coding experience required
