- The MDS Coordinator is responsible for the coordination of patient/resident care from admission through discharge.
- Starts and monitors RAI process tracking, discharge/re-entry and tracking forms through the PCC system
- Follows up with staff to assure compliance to standards of documentation
- Collect data for each resident and interviews staff and residents as necessary to assure good standard of practice and as instructed in the MDS 3.0 User manual
- Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient's care needs and captures all resources utilized to ensure correct payment by Medicare/Medicaid and insurance programs
- Provides interdisciplinary schedule for all MDS assessments and care plans
- Participating in daily Interdisciplinary Team Meeting and other meetings such as PDPM meetings, Care plan meetings, Triple Check Meetings
- Establishing and maintaining positive relationships with all members of the healthcare team and will function as part of the nursing management team.
- Must have a current, unencumbered LVN or RN license in Texas
- Must have a minimum of 1 year of direct hands-on nursing experience
- Must have a minimum of 1 years of long-term care experience
- Minimum 1 year of RAI/MDS experience
- Proficiency in ICD 10 coding experience required
