- Identify issues by evaluating the medical and psychosocial status of patients, considering various factors such as environmental, financial, and cultural influences.
- Collaborate with physicians, providers, patients, families, and payers to ensure high-quality, efficient, and timely care continuum.
- Enhance health outcomes and quality of life, while minimizing complications and costs for individuals with chronic health conditions.
- Empower patients to self-manage chronic health issues through tailored assessments and education.
- Effectively communicate with insurers, physicians, patients, families, and other health care providers, showcasing negotiation skills with assertiveness.
- Maintain professional, calm, and effective communication methods.
- Foster positive relationships by promoting understanding and goodwill among claimants, payers, providers, and professional staff.
- Hines and Associates STRICTLY employ licensed/certified medical professionals holding an unrestricted license/certification in the state where they operate or in a state with license reciprocity.
- Must be a registered nurse.
- Successful completion of a case management orientation program.
- Minimum of 2 years of experience in a full-time acute inpatient setting or specialized area (e.g., emergency department, inpatient behavioral health, or treatment program).
- Strong communication skills
- Basic typing and computer proficiency with a minimum typing speed of 35 WPM.
- Prior experience in case management is beneficial but not mandatory.
- Previous involvement in case management or insurance is advantageous but not obligatory.
