    Starting Compensation:

    Case Manager I: $48, $55,390.64 annual salary based on experience.

    Case Manager II: $54, $62,266.51 annual salary based on experience.

    Case Manager III: $56, $65,327.49 annual salary based on experience.

    Role:

    Under the supervision of the Mental Health Program Director and Program Coordinator, candidate will assess individual needs, identify goals, develop a person-centered plan for treatment, oversee the implementation of services and link and coordinate with other community agencies. Candidate to include integration of physical and behavioral health, all using the best practice, strength-based, person-centered model to be identified for the consumer.

    Full time 40-hour position as Supports Coordinator for children, adolescents and adults with intellectual and developmental disabilities. Includes support and collaboration with families, community agencies and school system. Performs annual assessments, develops person centered plans, advocates for and promotes self-advocacy. Differential diagnosis in collaboration with supervisor and other clinicians, maintains records per agency standards, provides psychoeducation, coordination of CMH and community services, adheres to confidentiality and recipient rights, develops natural supports, provides support to Program Coordinator and team with quality improvement activities.

    Knowledge of Intellectual/Developmental Disabilities primary and mental illness preferred. Prior experience working in a Community Mental Health system as a Supports Coordinator preferred. Experience providing clinical intervention to consumers living in community settings and specialized residential preferred. Experience working with electronic records. Ability to work as part of a team in a public mental health setting. Experience with providing therapy preferred.

    Valid MI driver's license required.

    Job Specific Requirements:

    Support Coordinator I: Bachelor's degree in social work or equivalent with LBSW or LLBSW required. If licensed at LLBSW level, individual must obtain full LBSW within two years of hire.

    Support Coordinator Level II: Master's degree in social work or equivalent with LLMSW or equivalent required. If licensed at LLMSW level, individual must obtain full LMSW within two years of hire.

    Support Coordinator Level III: Master's degree in social work or equivalent with LMSW, LPC or LLP.
