Nurse Practitioner/Physician Assistant to work in collaboration with physicians staff RNs and other healthcare professionals to provide high-touch compassionate care to patients and families.Proven ability to perform comprehensive adult geriatric admission history physical exam diagnose document episodic acute chronic disease in adults recognize conditions characteristics behaviors that place seniors at risk pursue feedback continuing education professional development effectively reconcile medications identify document needs clinically complex patients work collaboratively with physicians case managers social workers family members key caregivers ancillary medical personnel as appropriate participate Quality Assurance Performance Improvement activities assess medical necessity effectiveness ancillary services determine initiate benefit events communicate process providers team members coordinate care member transition different levels care care settings ensure ICP communicated immediately or later than hours receiving care setting maintain accurate compliant documentation electronic medical record software ensures diagnoses ICD-10 coded accurately documented appropriately support diagnosis visit reviews orders interventions appropriateness response treatment identify most effective plan align member needs wishes participate family conferences provide advanced planning education needed address pharmacy recommendations pharmacist communications residents expected present assigned facilities during work hours ensure facilities adequate face face coverage meet resident facility needs practice ethically accordance Scope Standards Practice profession Board Certification rotating on-call responsibilities after-hour coverage required travels between sites mandatory performs duties assigned Family Makes Us Stronger our family your family one family committed loving giving caring united making difference eager connect you Apply Now get started PruittHealth Equal Employment Opportunity employer all qualified applicants receive consideration without regard race color religion sex national origin disability veteran status
