The Registered Nurse (RN) in Home Health is responsible for providing and overseeing nursing care for patients in their residences as per physician directives and in compliance with regulations and agency policies.

• Works collaboratively with other healthcare professionals to coordinate the overall care plan, ensuring continuity of patient care through effective communication with physicians, agency staff, patients, families, and community resources, all in a Point of Care setup.
• Conducts initial and/or comprehensive nursing assessments, identifies primary care focus, develops the care plan following state-specific guidelines, and maintains accurate, thorough, and timely documentation as per physician instructions.
• Performs comprehensive patient evaluations at specified intervals and formulates appropriate care plans.
• Ensures patients fulfill home health eligibility criteria and medical necessity standards outlined by the payer source.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Current Driver's License, vehicle insurance, and reliable vehicle or access to public transportation

• TN: No additional state-specific requirements
